You can request this declaration in the GOV.CV app. To start, click "Apply online" and follow these steps:

Enter first name, last name and other required details
Indicate the place of birth
Select the country and place of birth
Enter the residency details
Select the start date of residency
Attach the documents
Attach an identification document and visa, if applicable
Finalize the request
